Why Gut Cleanses Don't Work
(They Actually Make You Worse)
What the latest research reveals about juice detoxes, parasite protocols, and trendy cleanses.
Every week in my clinic, I see people who've made themselves worse.
They come in with bloating, brain fog, dysbiosis, and a weakened gut barrier. And when I ask what brought them in, the story is always similar: they did a juice cleanse, or a parasite protocol, or some trendy gut detox they bought online. They thought they were healing their gut. Instead, they injured it.
The biggest misconception people have is that the gut works like a clogged pipe. You use a cleaner to flush it out. Done. But that's exactly wrong. Here's what's actually happening inside your body.

Your gut is a delicate ecosystem, not a sewer system.
Your gut contains 37 trillion microorganisms (bacteria, viruses, fungi) living in careful balance. This microbiota is not a problem — it's the foundation of your health. It's part of an intricate system that includes:
- Immune cells — 70% of your immune system lives in your gut
- Enteric nerves — your "second brain," directly communicating with your central nervous system
- The epithelial barrier — a delicate lining only one cell thick, controlling what enters your bloodstream
- Microbial metabolites — compounds your bacteria produce that regulate mood, immunity, inflammation
This is not a clogged pipe. This is a rainforest. And trying to "clean it out" with a juice detox is about as subtle and helpful as napalming a national park.
A 2025 Northwestern University study found that even a 3-day juice-only cleanse significantly increases inflammatory bacteria and decreases beneficial bacteria. The high sugar and complete absence of fiber fuels pathogenic organisms while starving the beneficial ones. The microbiota shift was so pronounced that researchers observed changes within 72 hours.
How juice cleanses and detox protocols injure your gut.
1. They Strip Out Fiber (And Beneficial Bacteria Starve)
Juicing removes the fiber from fruits and vegetables. Your beneficial bacteria (especially Firmicutes and Faecalibacterium prausnitzii) feed on fiber to produce short-chain fatty acids that reduce inflammation and fuel your intestinal cells. Without fiber, these bacteria starve. They die off. Your microbiota becomes dysbiotic.
2. They Flood Your System with Sugar (Pathogenic Bacteria Thrive)
A single glass of juice contains enormous amounts of sugar — more than you'd get from eating the whole fruit. Without the fiber to slow absorption, this sugar spikes your blood glucose rapidly. Pathogenic bacteria like E. coli, Proteobacteria, and inflammatory Firmicutes love sugar. They multiply. Your microbiota shifts toward dysbiosis in 3 days.
3. They Compromise Your Gut Barrier (And Gut Permeability Increases)
With dysbiotic bacteria increasing and beneficial bacteria declining, your intestinal barrier starts to break down. The tight junctions that control what enters your bloodstream become loose. Your gut becomes permeable. Endotoxins from dysbiotic bacteria cross the barrier. Systemic inflammation spikes. You now have a "leaky gut" — which ironically is often blamed on gluten or food sensitivities when it was actually caused by your cleanse.
4. They Disrupt Your Immune System (At the Worst Possible Time)
70% of your immune system lives in your gut, trained by your microbiota to distinguish between friend and foe. When you rapidly shift the microbial composition with a cleanse, your immune cells lose their training ground. Some activate excessively (food sensitivities spike). Others become under-responsive. You end up more vulnerable to infections and less able to control inflammation.

The same problem applies to herbal parasite cleanses and other trendy detoxes.
I see a lot of people do herbal parasite protocols or activated charcoal cleanses with the same logic: get rid of the "bad stuff" in your gut. But most of these people don't have parasites. They have dysbiosis, food sensitivities, and weak digestion. And harsh antimicrobial protocols? They indiscriminately kill both pathogenic and beneficial bacteria. You end up with a devastated microbiota.
The pattern is the same: you're trying to sterilize an ecosystem. You're not addressing the actual problem — which is usually dysbiosis, poor digestion, or a dysregulated immune system.
Most cleanse marketing is built on the myth that your body accumulates toxins that need to be "flushed." But your liver and kidneys are constantly detoxifying your body. There's no scientific evidence that juice cleanses or herbal detoxes remove toxins. In fact, the FDA warns that many commercial cleanse products contain illegal or potentially harmful ingredients.
How real gut healing works: tending a garden, not sterilizing a sink.
Real gut healing requires three things, and none of them involve stripping your gut bare:
1. Calm Your Immune System
Your immune system is likely activated and dysregulated. We need to calm it down — not with immunosuppression, but by removing triggers (inflammatory foods, chronic stress), supporting the gut barrier, and using herbs and foods that have immunomodulatory effects.
2. Regulate Your Nervous System
Your enteric nervous system is dysregulated. Your vagal tone is compromised. Digestion only works optimally when you're in parasympathetic (rest-and-digest) state. This means: stress management, breathing practices, sleep optimization, and nervous system support. Chronically activated stress hormones wreck gut motility, barrier function, and microbiota composition.
3. Restore Microbial Diversity
Your microbiota is dysbiotic. But you don't fix dysbiosis by killing everything. You fix it by creating the conditions for beneficial bacteria to thrive: high-fiber foods (prebiotics), fermented foods (probiotics), properly digested protein, and targeted herbal support to restore barrier function and reduce inflammation. You're tending a garden, feeding the soil, not scorching it.
This takes time — real healing usually takes 8–12 weeks minimum. But the results are sustainable because you've actually restored the underlying ecosystem instead of creating a temporary (and damaging) shock to the system.
